We are a team of transplant co-ordinators (based at Lister Hospital) providing specialist nursing services for patients, their families and carers both before and after a kidney transplant. This includes transplants involving a living donation or deceased donation. We work closely with the teams from our three transplant provider hospitals:

Pre-transplant team
We provide all the preparation work, care and support for patients and families before they have a transplant. This includes organising the investigative procedures required, managing waiting lists in partnership with our three transplant hospitals, supporting our patients with all the information they need and offering guidance to patients who are interested in living donation.

Post-transplant team
We provide a follow up service for patients once they have had a transplant including weekly transplant clinics with consultant nephrologists, renal registrars, clinical nurse specialists, our renal pharmacist, dietician, and renal administrator. We provide full support to patients, responding to any questions or concerns related to their care.
