What is Shared Care Record?
A shared care record is a safe and secure way of bringing separate records from different health and care organisations together digitally in one place. The Shared Care Record helps staff to make the best decisions by having a more joined-up picture of a patient’s care. Information is available at the point of care throughout a patient’s journey.
Who can access Shared Care Record?
All practice staff who have access to patient records in SystmOne or EMIS can automatically open the Shared Care Record for any patient where direct care is being provided. The Shared Care Record is not the full record for an individual but brings together a valuable summary of information meaning you no longer have to chase departments for simple queries.
What can I see for East and North Hertfordshire NHS Trust?
Information available in real-time includes:
– upcoming appointments
– admissions
– pathology results (excluding cellular pathology)
– endoscopy and radiology reports
– inpatient and ED discharge summaries.
How do I access Shared Care Record
