Children’s Continuing Care

We have a team of highly experienced and skilled support workers who provide specialist nursing care under the direction of two Registered Paediatric Nurses to children with complex health needs in the Community.

The team is based at QEII Hospital and covers all of East and North Hertfordshire and provides nursing care 7 days a week with an out of hour on call service.

About our Service

The Children’s Continuing Care team aim to enhance quality of life, and empower and support families caring for children and young people with complex health needs by providing respite within the home.

Eligibility for continuing care

The decision support tool sets out the children’s needs across 10 domains, divided into different levels of need. Eligibility is met when the level of need is assessed as high within three domains, or severe or priority in one domain. The nominated health assessor will use their skill, expertise and evidence-based judgement to consider what, for each domain, is over and above what would be expected for a child or young person of that age.


Training High

The team also provides specialised training in a variety of settings such as education/residential and day care settings in order to meet the needs of children who require clinical nursing care within these environments.

Training can consist of one off information giving sessions or comprehensive theory and practical training packages including assessment of competency where appropriate. For more information, click on the links below:
