Palliative and end-of-life care services

Palliative care describes care given to relieve symptoms rather than treat the cause of an illness, the aim of which is to improve the quality of of patient’s life who has a life-limiting illness.  Palliative care can help at all stages of such an illness, from its diagnosis, during on-going treatment and at the end of someone’s life.

The Trust’s specialist palliative care team work solely with patients who have life limiting illnesses, help people with:

  • Advice on symptom management
  • Advice on living with any life limiting illness
  • Holistic assessment and advance care planning
  • Information to patients and families, including financial matters
  • Emotional and psychological support to patients and families
  • Discharge planning
  • Referrals to community services – for example district nurses, community palliative care specialists/hospice services
  • Bereavement support
  • Support for patients and families in the last days of life

The palliative care team works closely with other professionals and services to ensure that your care is as seamless as possible.

Lister service

The service at the Lister runs 9.00am to 5.00pm, every day of the week (including public holidays).  The team consists of:

  • Clinical nurse specialists (CNS)
  • Consultant in palliative medicine
  • Social work practitioner
  • End-of-life care lecturer practitioner
  • Palliative care lecturer practitioner
  • Multi-disciplinary team (MDT) co-ordinator
  • Administration support

Mount Vernon Cancer Centre service

The palliative care service at the Mount Vernon Cancer Centre is available Monday to Friday, 9.00am to 5.00pm.  The team comprises:

  • Clinical nurse specialists
  • Consultant in palliative medicine
  • Administration support
